Your Admission

What You Will Need In The Hospital
Items that you may want to bring with you include:
  • A list of medications you are taking.
  • Current medical insurance information/cards.
  • Simple toiletries, robe and slippers (if you do not have your robe and slippers and other needed items, the Hospital has them available for your use).
  • Do not bring valuables such as jewelry, watches, or a large amount of cash to the Hospital (we encourage you to limit the amount of money at the bedside to $5.00 or less). 

Patient Valuables
Valuables that have been brought to the hospital can be placed in a safe in the Security Department. You are encouraged to either place your valuables in the safe or send them home. The Hospital is not responsible for lost valuables or belongings. The patient accepts full responsibility for any personal belongings taken to the hospital.

Advance Directives 
If you have an advance directive, durable power of attorney for health care, mental health care or a living will, please bring a copy of it with you to the hospital for placement on your medical record. If you would like information regarding any of these documents, please request that Social Services comes to see you.

Insurance Information 

In order to make sure you receive all the financial payment benefits you are entitled to, please bring all insurance information, including special forms, certifications, and authorizations. If your admission to the hospital requires pre-approval by your primary care physician or insurance company, please verify that this has been done. If this has not been completed, please call 270-7991 and let us know if we can assist you. If you are covered by your spouse's insurance, please bring that insurance information as well. This information will be used to bill your hospital stay to the appropriate party. 

If you have been involved in an automobile accident, or the procedure you are having is covered by your auto insurance, please bring your auto insurance card that includes your policy number and insurance agent's name and phone number, and any health insurance information.

If your admission is due to a work-related injury, please indicate this information and provide the name of your employer and insurance carrier. You are encouraged to contact your insurance company to have your insurance questions answered.
