Sleep Disorders Center - Frequently Asked Questions
I have an appointment scheduled. Are there any forms I need to bring with me?
The following forms are
used by the Sleep Disorders Center to process all new patients. If you are
already scheduled for a sleep study, please complete all the forms which require
information from you and/or your bedpartner and return them to: GSH Sleep
Disorders Center, 206 Hathaway Park, Lebanon, PA 17042.
Completed forms can also
be faxed to the Sleep Disorders Center at 717-270-2242.
If you do not yet have
a sleep study scheduled, please call the Sleep Center at 717-270-2349 to
schedule one and for instructions on how to proceed.
