Job Description:
Provides Physical Therapy services including patient evaluation (infant, child, adolescent, adult, older adult), develops and administers treatment plan, documents and communicates with physicians and other referral sources and provides clinical guidance to other staff.

Required Qualifications:
Bachelor's degree from a Commission on Accreditation of PT Education (CAPTE) accredited college program.
