Insurance Information In order to make sure you receive all the financial payment benefits you are entitled to, please bring all insurance information, including special forms, certifications, and authorizations. If your admission to the hospital requires pre-approval by your primary care physician or insurance company, please verify that this has been done. If this has not been completed, please call 270-7991 and let us know if we can assist you. If you are covered by your spouse's insurance, please bring that insurance information as well. This information will be used to bill your hospital stay to the appropriate party. If you have been involved in an automobile accident, or the procedure you are having is covered by your auto insurance, please bring your auto insurance card that includes your policy number and insurance agent's name and phone number, and any health insurance information. If your admission is due to a work-related injury, please indicate this information and provide the name of your employer and insurance carrier. You are encouraged to contact your insurance company to have your insurance questions answered.
